Estudo randomizado | Bulevirtida reduz os níveis de HDV RNA e TGP em pacientes com hepatite D crônica

Resumo: Este ensaio clínico randomizado de fase 3 avaliou a eficácia da bulevirtida, um inibidor da entrada do vírus da hepatite D (VHD) em hepatócitos, em pacientes com hepatite D crônica. Os pacientes foram randomizados para receber 2 mg ou 10 mg de bulevirtida diariamente por 144 semanas, ou nenhum tratamento por 48 semanas seguido de 10 mg de bulevirtida diariamente por 96 semanas. O estudo envolveu 150 pacientes, sendo 49 no grupo de 2 mg, 50 no grupo de 10 mg e 51 no grupo controle.

Após 48 semanas de tratamento, 45% e 48% dos pacientes nos grupos de 2 mg e 10 mg, respectivamente, atingiram o desfecho primário de nível indetectável de HDV RNA ou uma diminuição de pelo menos 2 log10 IU/mL em relação ao baseline, e normalização do nível de TGP. Apenas 2% do grupo controle alcançou o desfecho primário. Os níveis de TGP normalizaram em 51% e 56% dos pacientes nos grupos de 2 mg e 10 mg, respectivamente, uma diferença significativa em relação à normalização de 12% no grupo controle. Vale notar que a perda de HBsAg não ocorreu até a semana 48 nos grupos bulevirtida. Não foram relatados eventos adversos graves relacionados ao tratamento, embora efeitos colaterais, incluindo cefaleia, prurido e fadiga, tenham sido mais comuns nos grupos bulevirtida.

Esses resultados demonstram a eficácia da bulevirtida na redução dos níveis de HDV RNA e TGP em pacientes com hepatite D crônica. No entanto, a ausência de perda de HBsAg nos grupos bulevirtida levanta questões sobre seus efeitos de longo prazo.

O CDC recomenda rastreamento para o vírus da hepatite B pelo menos uma vez na vida para todos os adultos com idade ≥18 anos

Resumo: O Centers for Disease Control and Prevention (CDC) emitiu novas recomendações para rastreamento para o vírus da hepatite B (HBV) nos EUA.

As recomendações incluem rastreamento para infecção pelo HBV pelo menos uma vez na vida para adultos com idade ≥18 anos e testes mais frequentes para pessoas com risco aumentado de infecção pelo HBV. Os grupos de risco incluem:

  • Pessoas encarceradas ou anteriormente encarceradas em prisões ou outras instituições de detenção.
  • Pessoas com histórico de doenças sexualmente transmissíveis ou múltiplos parceiros sexuais.
  • Pessoas com histórico de infecção pelo vírus da hepatite C.

O CDC recomenda o uso do painel triplo (HBsAg, anti-HBs e anti-HBc total) para triagem inicial para ajudar a identificar pessoas que têm uma infecção ativa pelo HBV, tiveram uma infecção resolvida e podem estar suscetíveis à reativação, são suscetíveis e precisam de vacinação ou estão vacinadas.
